It’s easy. Now that you have selected your UniMed Health Plan simply complete the Membership Application Form and Direct Debit form and either send them by mail, email or upload them below.
Please take your time and fill in all the required sections, accurately and completely. This means we will be able to assess your application promptly. It normally takes 5 working days to process Membership Applications. However, if we need further information from you, this may take a bit longer to complete.
Your cover will commence on the date we accept your application and this will be indicated on your Membership Certificate. If any symptoms, diagnosis or treatment occurs between you applying and being accepted, you must inform us of this.
