Your Health Plans' Terms, Conditions and Eligibility Criteria vary depending on whether it was originally issued under UniMed or Accuro.

Is your Health Plan originally issued under Accuro? If you are on a SmartCare, SmartCare+, StaffCare, StaffCare+, SmartStay Health Plan, or another Health Plan originally issued under the Accuro brand, the terms and conditions are contained within your Health Plan document. You can find it in your Member Portal.

HealthCarePlus

The fastest and easiest way to submit a claim is through Unimed's Member Portal. Submit claims as you go, there's no need to wait to submit a bulk lot of claims together.

These forms are for Members on Health Positive, Hospital Select Plus Modules, UniCare Advantage, other Health Plans originally issued by UniMed, or group insurance schemes through your employer issued by UniMed. For plans originally issued under Accuro, click here for your forms.
